What we know about the Amazon cargo plane that killed 5 at Miami International Airport

Federal investigators are examining whether an Amazon cargo jet touched down too far along the runway before barreling off the pavement at Miami International Airport and crashing into vehicles, killing at least five people.

The Boeing 767 overran the runway around 2 p.m. Sunday after arriving from San Juan, Puerto Rico, before crossing a roadway used by warehouse workers and businesses surrounding the airport.

Five people were killed, three were critically injured, and two others were hospitalized with less severe injuries, officials said.

The National Transportation Safety Board has dispatched a team led by Chairwoman Jennifer Homendy to Miami to investigate the crash. The agency is expected to provide its first briefing on the investigation Monday.

One of the first questions investigators are expected to examine is exactly where the cargo plane touched down.

Video posted online appears to show the aircraft remaining airborne well down the runway before finally landing. Aviation experts say investigators will want to determine whether the plane touched down beyond the normal landing zone and, if so, why the pilots continued the approach rather than performing a go-around and attempting another landing.

The typical touchdown zone is within the first 3,000 feet of a runway, according to aviation experts.

Aviation safety expert Steve Arroyo said that if the aircraft was significantly beyond that area, the crew should have abandoned the landing and gone around for another attempt.

Investigators are also expected to examine the aircraft’s speed, braking performance, weather conditions, winds, mechanical systems and maintenance history before determining what caused the deadly overrun.

Mary Schiavo, a pilot and former inspector general for the Department of Transportation, said video of the landing appears to show the aircraft floating above the runway for an extended period.

Schiavo said investigators will likely examine whether wind conditions contributed to the aircraft traveling farther down the runway before touching down.

It was not raining at the time of the crash, but dark storm clouds were nearby and strong winds had been reported.

“A tailwind would certainly add to your troubles if you’ve already given up a good chunk of the runway by not touching down and landing right away,” Schiavo said.

The investigation will ultimately determine whether wind played any role in the crash.

The aircraft plowed beyond the airport property and across a nearby roadway before coming to rest near two semitrucks.

Images from the scene showed the plane largely intact and resting on its belly despite a significant fire.

Miami-Dade Fire Rescue Chief Ray Jadallah said the pilot and copilot were trapped inside the cockpit following the crash, while other people were trapped inside vehicles.

Firefighters also had to rescue a person trapped underneath a vehicle, according to the department.

It was not immediately clear whether either pilot was among those killed.

Roughly 200 emergency personnel responded as heavy flames and thick black smoke poured from the aircraft.

Firefighters encountered what the department described as a complex engine compartment fire and used foam from specialized airport firefighting vehicles to bring the flames under control. Crews were still dealing with a fuel leak from the aircraft hours later.

Investigators could also examine whether additional runway safety equipment might have changed the outcome.

Miami International Airport does not have an Engineered Materials Arresting System, or EMAS, at the runway involved. The systems use beds of lightweight, crushable material designed to slow an aircraft that overruns a runway.

The Federal Aviation Administration says the systems have been installed at more than 120 airports and have helped safely stop overrunning aircraft carrying hundreds of people.

However, former FAA director of airport safety and accident investigations Mike O’Donnell said Miami appears to meet federal requirements without an arresting system because it has a 1,000-foot safety area beyond the runway.

Runway safety requirements were strengthened following the 1999 crash of American Airlines Flight 1420 in Little Rock, Arkansas. That aircraft overran a runway during a severe thunderstorm, killing nine people.

The Amazon Prime Air flight involved in Sunday’s crash was operated by 21 Air, a North Carolina-based cargo carrier.

The Boeing 767 was approximately 32 years old and was originally manufactured as a passenger aircraft, according to flight-tracking service Flightradar24. It flew for several airlines for more than two decades before being converted into a cargo aircraft in 2015.

Schiavo said investigators will closely review the plane’s maintenance records and operating history.

The age of an aircraft alone does not establish that it is unsafe, and investigators have not determined that a mechanical problem contributed to Sunday’s crash.

The NTSB investigation is expected to include an examination of the aircraft’s flight data recorder and cockpit voice recorder, if recoverable, along with maintenance records, air traffic control communications, pilot actions and environmental conditions.

The crash caused significant disruptions at one of the nation’s busiest airports.

Flights were halted for much of Sunday afternoon as emergency crews responded. More than 160 flights were canceled and nearly 325 were delayed by late Sunday, according to FlightAware.

Some flights were diverted more than 200 miles north to Orlando International Airport.

Amazon said it will cooperate with the investigation and expressed condolences to those affected.

“We’re heartbroken to learn that five people lost their lives in today’s incident at Miami International Airport,” the company said. “Our deepest sympathies go out to the families, loved ones, and all those affected by this devastating loss.”
